To submit a Chapter Meritorious recipient, or to nominate an individual for a National award, enter information in the fields below, then click on Continue to proceed to Step 2. The spelling entered for the nominee's name will be used on the award.

To nominate a Chapter for the Give Chapters a Hand Awards, enter information below (leaving the Name, Phone and Email fields blank), then click on Continue to proceed to Step 2.
